Review fixes for smart casts in / out loops.

LoopTypeInfo became TypeInfoWithJumpInfo rewritten in Kotlin.
Fixed a bug with break inside Elvis inside a loop, a pack of related tests added.
isTrueConstant moved to JetPsiUtil.
